This is not a maturity model. It is a set of questions designed to reveal whether your experimentation and research infrastructure is actually doing what you think it is doing.

NOTE:Several of these questions ask whether your systems enforce something. A required field that accepts 'we think this will be better' is not enforcement. It is a text box with an asterisk. If your instinct is 'well, sort of' or 'in theory, yes' or 'the good teams do,' that is a workspace, not infrastructure. The difference between the two is what this diagnostic measures.
